Can't submit forms (cancel sub/contact us)

Submitting forms generates a no url's allowed page. I'm trying to cancel my sub. The contact us form does the same thing.

Which "contact us" form are you using, i.e., from where?
Which browser and computer (or phone) are you using, and do you get the same errors on another browser?
